Primary care providers at Southwest Medical Center can help you with all your primary care needs. More importantly, as you continue to develop a relationship with your primary care provider, the two of you work together to help you become a healthier version of yourself.
Primary Care is the entry point into the health care system. Primary care clinics are the point of delivery of individual care, based on care over time.
Services from a sore throat to, smoking cessation counseling and weight-loss programs; diagnostic services, such as lab tests, EKG’s and imaging; and treatment.
We also manage chronic health conditions. A chronic conditions is a health issue that may last at least 12 months, and which places you at significant risk of functional decline, health risk or death. Examples include arthritis, asthma, diabetes, COPD, and heart disease.
