A great primary care provider will help in managing your long term health needs. We want you to stay healthy and independent.
Preventive Care & Support
Preventive care is care you receive to prevent illness, detect medical conditions, and keep you healthy. At SouthwestMed we offer many preventive services, such as screenings, vaccines, and counseling. If you meet the eligibility requirements and guidelines for a preventive service, your provider will proactively discuss this with you.
Preventive services recommended by the U.S. Preventive Services Task Force are typically covered by your insurance or Medicare.
During the course of your preventive care, your provider may discover and need to investigate or treat a new or existing problem. This additional care is considered diagnostic, meaning your provider is treating you because of certain symptoms or risk factors.
Outpatient Care & Support
Primary care providers at SouthwestMed can help you with all your primary care needs. More importantly, as you continue to develop a relationship with your primary care provider, the two of you work together to help you become a healthier version of yourself.
Primary Care is the entry point into the health care system. Primary care clinics are the point of delivery of individual care, based on care over time.
Services from a sore throat to, smoking cessation counseling and weight-loss programs; diagnostic services, such as lab tests, EKG’s and imaging; and treatment.
We also manage chronic health conditions. A chronic condition is a health issue that may last at least 12 months, and which places you at significant risk of functional decline, health risk or death. Examples include arthritis, asthma, diabetes, COPD, and heart disease.
Transitional Care & Support
Managing your care at home after a hospital or skilled nursing facility (SNF) stay can be confusing and difficult. At SouthwestMed we offer care coordination (also called transitional care management) if you need assistance organizing your care after leaving a facility. As your primary care provider, we automatically provide care coordination, or you can request services by asking that your hospital or SNF notify your PCP about your discharge home.
You should receive an in-person visit from your provider within 7 or 14 days of your return home, depending on the complexity of your condition. We offer the following services, as necessary:
- Contact you within two days of leaving the hospital or SNF
- Work with your other health care providers to provide education and other services to help you transition back to living at home
- Review your medications and any changes that have been made.
- We review the need for follow-up visits and help you schedule them
- Identify medical needs you have and arrange referrals to follow-up care and other community resources
